Information for NEW Patients

Becoming a new patient of Suburban Gastroenterology is very simple.  If you know your insurance requires a referral, please contact your primary care physician before scheduling your appointment.  If you already have a referral, please fax it to us at (630) 527-6456 or bring it with you at the time of your appointment.

You should also complete a New Patient Packet.  Simply download and complete the PDF file and bring it with you on your first visit.

Most procedures and treatments can be performed on site at Suburban Gastroenterology or at its ambulatory surgery center, Midwest Endoscopy Center.  However, for those times where a hospital stay may be needed, Suburban Gastroenterology’s team of board certified physicians have privileges at the following hospitals:
